In order to lock your loan, please fill out the following form in detail, list all of your fees, as it will appear on the loan documents. Make a note that any changes after the loan documents have been drawn; there will be additional $200 charge to redraw the loan documents.

 For Impound Accounts please provide the following documents: 

  • Complete Estimated HUD 1
  • Evidence of Hazard Insurance

Please note that the loan documents will not be drawn if the above items are not provided for the impound accounts including the appraiser’s phone number.

Invoices must be provided for all the fees charged for all third parties.
